Please read this document carefully. This is a rock-climbing event sponsored by United Rocks, a non-profit which mission is to promote physical, mental and social skills through the sport of rock climbing for people with intellectual and developmental disabilities. Movement Climbing Gym is a hosting facility for the event taking place on March 19th, 2023 (referred to in this document as “gym”). The activities to be performed during rock-climbing events sponsored by United Rocks, include artificial wall climbing on fixed walls (including top-roping).
In compliance with State and Federal regulations, any adult or parent or legal guardian of a minor (referred to in this document as “parent”) must sign this document, as evidence of their agreement to these terms and conditions, for themselves and, to the maximum extent allowed by law, on behalf of the minor participant.

The “parent” acknowledges and agrees with the following:

Are responsible for managing the risks of the activities and the premises, supervised or not, and must follow all “gym” policies and rules.
Hereby acknowledges the risks associated with the activities described above, and that other risks, inherent and otherwise, may be encountered, and expressly assumes such risks, on behalf of him/herself and of the minor, and whether or not described above.
Agrees for his/herself and, to the maximum extent allowed by law, on behalf of a minor visitor of whom he/she is the parent or guardian), to release and discharge United Rocks, its owners, staff and volunteers, from all claims, liabilities, and losses asserted by or on behalf of me or the minor visitor in any way arising from or connected with their visit to Movement Climbing Gym on 03/19/2023, the use of its equipment and facilities, or otherwise being on the premises of the gym on such date. I understand that by signing this document, I surrender my, and the minor visitor’s, right to make a claim or file a lawsuit against United Rocks, its owners, staff and volunteers for personal injury, illness, property damage, wrongful death, products liability or any other theory, to the maximum extent allowed by law.
I further agree to hold harmless and indemnify (that is, defend and pay or reimburse) United Rocks, its owners, staff and volunteers from any claim and from any liability, loss, damages or expenses (including attorneys’ fees and insurance deductibles) resulting from 1) a claim brought by a participant or any other person for loss or damage caused by my or the minor participant’s acts or omissions; and 2) a claim brought by any member of my or the minor participant’s family in any way arising out of my or the minor’s participation in an activity sponsored by United Rocks, the use of equipment and facilities, or otherwise moving about the premises of the gym.
In the event that I should require medical care or treatment, I authorize United Rocks, its owners, staff or volunteers to seek immediate medical attention and I agree to be financially responsible for any costs incurred as a result of medical attention or treatment. I am aware and understand that I should carry my own health insurance.

I have carefully read, understand and voluntarily sign this agreement and acknowledge that it shall be effective and binding upon me, my minor child or ward who is a participant, and my, or the minor’s, family, heirs, executors, administrators, and representatives.
